2024-2025 Activity Card Submission
NIAA activity cards are available for $50.00 each for superintendents, school trustees, principals, school administrators, deans, athletic administrator, athletic directors (if applicable), cheer advisor, NASC advisor, head coaches, and assistant coaches of sanctioned activities, officials, and/or other individuals deemed important to high school athletics in your respective school. 

This card should not be issued to parents, boosters or others not directly involved with athletics. The card admits the holder and one guest to any interscholastic activity sponsored by this Association or any member school of this Association. 

Please note the following information:

- Cards should be ordered by an authorized school administrator (superintendent, principal, athletic administrator). A copy of the order should be included with the payment check. You can choose to receive a copy of your answers via email after completing this form.

- Activity cards are non-transferable. Only one card will be issued per person, even if they hold positions at two different schools and/or districts.

Please mail a check to the NIAA office for the total amount.
